DMP

DMP = Disease-Management-Programm

Das DMP ist ein systematisches Behandlungsprogramm für chronisch kranke Menschen. Die gestzliche Krankenversicherung (GKV) bezeichnet diese Programme auch als strukturierte Behandlungsprogramme oder Chronikerprogramme. Ziel ist es, den Bundesbürgern, die an chronischen Erkrankungen leiden, eine kontinuierliche ärztliche Behandlung, Überwachung und medikamentöse Therapie zu ermöglichen. Der Hausarzt ist Ansprechpartner und übernimmt die Langzeitbetreuung. Der Patient sollte sich mindestens einmal im Quartal beim behandelnden Arzt vorstellen.

Ziel ist eine langfristige, teilweise präventive Begleitung des chronisch kranken Patienten.

Wertigkeit:

  • Das DMP dient der abgestimmten und kontinuierliche Betreuung und Behandlung vor Folgeerkrankungen. Koordination der Zusammenarbeit von Haus- und Fachärzten sowie Krankenhäusern, Apotheken und Reha-Einrichtungen. Ermöglichung von Therapieschritten nach wissenschaftlich gesichertem medizinischem Wissensstand und aufeinander abgestimmte (evidenzbasierte) Medizin. Kostensenkung (mittel- bis langfristig) der Leistungsausgaben der Krankenkasse.
